Healthy Ageing Clinic

Our fantastic and caring nurses can visit people 75 years and over in their own home to assist in planning and preparing for the future. The nurses will work with you and your GP to help you stay well living in your own home.


You may feel all your needs are being met at present, but our nurses can discuss some options you may require for the future.


When you feel your needs are increasing, we will help you access services and get them in place when the time is right.

Planned Care

Planned care is great if you have a long-term health condition such as arthritis, diabetes, asthma, heart disease (and many more). Your GP and Nurse will discuss with you the benefits of planning your care needs if you have a long term or ongoing health condition. They can help you plan your better health with a GP Management Plan (Care Plan). Not planning leads to “reactive care” which is treating you only when you are unwell. The aim of planned care is to prevent this from happening and reduce the chance of you needing to go to hospital.


A GP Management Plan is a written health plan, which identifies and outlines YOUR goals for the management of YOUR health, plus plans any treatments, service and care, to help you achieve these goals.


This plan includes timelines and regular reviews.


Your GP works with you and our nurses to write the plan. Together they will help you to determine your goals to live well as well as anyway, we can assist you to reach these goals.


We can help you access other health professionals (pharmacists, podiatrists, exercise physiologists, physiotherapists, occupational therapists, dietitians, medical specialists) as well as community services.

You can share your GP Management Plan with your other health providers so together they are all working towards the same goals.

Women’s Health

Our Registered Nurse Janette is authorised to perform pap smears/ cervical screening tests.


She will work with your GP so that you have your test when necessary and will answer any questions you have.

Heart Health Check

One fifth of Australians aged 45–74 years have a high chance of having a heart attack or stroke in the next five years.


Anyone 45 years and over or 30 years and over for Aboriginal and Torres Strait Islander peoples should have a regular Heart Health Check.
